APPOINTMENTS
Our appointment system allows you to speak directly to your physician’s secretary to make your appointment. Please call Monday – Friday 8:30 a.m. to 12:30 p.m. and 1:30 p.m. to 5:00 p.m. Please see our contact page for phone numbers.

New patients: for your convenience, our patient registration form is available online for you to fill out prior to your first office visit, regardless of which physician you are seeing. Complete the information as indicated and bring it with you to your first appointment. Please note that these forms cannot be submitted electronically.
At your first visit our medical group requires photo ID and your insurance card. If any additional information is necessary, our reception staff will happily assist you further. Our medical group does not verify insurance or benefits.

All patients must read and sign the following documents upon arriving for their first visit:

Cash patients are eligible for a “prompt pay” discount. To receive the discount, fill out the Prompt Pay/Cash Discount agreement

Advance Directives are not furnished by our medical group, but can be obtained from the California Medical Association.

CANCELLATION POLICY
If you are unable to keep your appointment, kindly give us 24-hour notice, or you will be subject to a missed appointment fee. We appreciate your courtesy as it may allow us to accommodate other patients with urgent medical needs.
