APPOINTMENTS

Tarzana Endocrine and Medical Group's appointment system allows you to speak directly to your physician’s secretary to make your appointment. Please call Monday – Friday 8:30 a.m. to 12:30 p.m. and 1:30 p.m. to 5:30 p.m.  Please see Contact Us for phone numbers.

New patients: for your convenience, our patient registration form is available online for you to fill out prior to your first office visit, regardless of which physician you are seeing. Complete the information as indicated and bring it with you to your first appointment.
Please note that these forms cannot be submitted electronically.
At your first visit our medical group requires photo ID and your insurance card. If any additional information is necessary, our reception staff will happily assist you further. Our medical group does not verify insurance or benefits.

All patients must read and sign the following documents upon arriving for their first visit:

• Patient Information Sheet
• Notice of Privacy practices
• Acknowledgement of Notice of Privacy Practices
• (If applicable) Authorization for Release of Medical Information
• Advance Directives are not furnished by our medical group, but can be obtained from the California Medical Association.

CANCELLING AND RESCHEDULING YOUR APPOINTMENT

If you need to cancel or change your appointment with one of our physicians, please give us 24-hour notification so that we can reschedule your visit for a more convenience time. Your physician’s secretary can help you with this. We appreciate your courtesy as it may allow us to accommodate other patients with urgent medical needs.
